Communicate with patients and families in an effective, patient-centered manner, utilizing techniques such as motivational interviewing and the teach-back method.Support patients and families in self-management, self-efficacy and behavior change.Provide copy of clinical summary to patients.Ensure all needed medical records are available to provider prior to patient’s office visit.Provide clinical advice to patients by phone or email per practice protocol.Ensure that patient medical records are up to date with information on specialist referrals, consults, hospitalizations, ER visits and community organization related to their health.Provide results to clinicians (per practice protocol) and make certain results are logged into the patient’s medical record. Track imaging and lab tests until all results are received. ![]()
